Welcome to Montana State Fund's Online First Report of Injury (FROI).
Injured Employee: The online First Report of Injury service is for employers only. Please report your injury to your employer. If this is not possible, please contact an MSF customer service specialist at (800) 332-6102 between the hours of 8:00am - 5:00pm (Monday - Friday) and they will take your claim over the phone.
Employer: The online First Report of Injury service is a quick, easy, and confidential way for employers to submit claim information in a step-by-step process. It also allows Montana State Fund to receive the claim information faster, which helps us better manage the claim. Montana law requires employers to file a full and complete report of every on-the-job accident, injury and/or occupational disease (OD) to an employee arising out of or in the course of employment within six days after notice. A customer service specialist will contact you within one business day if additional information is necessary to complete your claim. To complete a FROI, you will need several pieces of required information. First, you must have information about the Employer

After submitting the claim with the minimal requirements, an MSF customer service specialist will contact you if additional information is needed. You also have the option to fill out the form in its entirety. |
Montana State Fund’s systems will be down from approximately 10:00pm to 11:00pm Monday-Friday and as needed for emergency maintenance. During those down times, FROI’s may not be entered online. |
After successfully submitting the FROI, the system will display your MSF claim number information which you should reference when communicating with Montana State Fund. You will also have the option to print a copy of the FROI. Please be aware that MSF will investigate the claim, and the claim number does not constitute acceptance of liability. |
If you did not already login from the Home page, you may login from here by clicking the arrow in the top right corner of the page and entering your User ID and Password. By being logged in you have the ability to partial save your data at any time and come back to it within five working days of starting the report. If a complete report is not submitted within five working days, it will be deleted from the system. Partial save does not constitute filing a claim. You will also have the option to print a copy of the FROI. When you return to submit your form, log in and go to the First Report section of your account, view and select your report from the Incomplete FROI list, complete the form and click submit. After successfully submitting the FROI, the system will display your MSF claim number information which you should reference when communicating with Montana State Fund. Please be aware that MSF will investigate the claim. The claim number does not constitute acceptance of liability. |
