Automatic Withdrawal
With our automatic payment option, you can pay your premiums without writing a check. Your payments are automatically deducted as they become due based on your invoices.
The amount withdrawn from your account will be any balance due on your most recent invoice. This may include estimated premium installments or premium due after you have reported your payroll. You'll receive an invoice several days before the withdrawal is to take place.
To get started, complete the Automatic Withdrawal Authorization Agreement and return it to us via:

Please note: If you would like to change or discontinue your electronic funds transfer agreement, please notify us in writing and we will accommodate your request.
Note: The submit button used to email the automatic withdrawal authorization is only available if the form is opened in Adobe. You may need to configure your web browser to download the form in Adobe instead of opening the form in the web browser window:
- Chrome Browser Configuration:
- 1. Open a new session of Chrome and copy and paste the following link into the address bar: chrome://settings/content/pdfDocuments
- 2. Select "Download PDFs" if it is not selected already
- 3. Exit Chrome and re-open
- 4. Downloaded PDF files will now open in Adobe
- 5. Click on Automatic Withdrawal Authorization link (listed above).
- Microsoft Edge Browser Configuration:
- 1. Open a new session of Microsoft Edge and copy and paste the following link into the address bar: edge://settings/content/pdfDocuments
- 2. Move the slider for "Always download PDF files" to the right so it shows in dark blue
- 3. Exit Microsoft Edge and re-open
- 4. Downloaded PDF files will now open in Adobe
- 5. Click on Automatic Withdrawal Authorization link (listed above).
